Haleyville Rotary Toys for Kids Accepting Applications

Applications are being accepted through November 25. Qualifications for applications:

1. You must be a parent or legal guardian of the child.
2. You must provide proof of residence.
3. Only one application per child allowed.
4. Birth Certificate or proof of guardianship will be required.
5. All applications must be received by the deadline of November 25 at Noon.
6. No late applications will be accepted.
7. All toys must be picked up Friday, December 12, 2014 from 5:00 – 8:00 PM at the American Legion Building.

Applications for Toys for Kids can be picked up at these locations:

• Alabama Power – 8:30 – 5:00
• Traders & Farmers Bank – 8:30 – 5:00
• First National Bank – 8:30 – 5:00
• Pinnacle Bank - 8:30 - 5:00
• Lakeland Community Hospital Emergency Admitting Department – 24 hours/day

If you would like to sponsor a child you may do so for just forty ($40) dollars.  You are requested to purchase one large toy and two smaller toys for the child you have selected. You can pick up a star at Lakeland Community Hospital with the child’s age and sex listed. (Please see Katrina Wray or Whitney Howe)

The Rotary Club is accepting monetary donations and any new toy for this year’s program. You can bring your donations by Alabama Power, First National Bank, Traders & Farmers Bank, Pinnacle Bank or Lakeland Community Hospital. You may also mail a check to Lakeland Community Hospital, PO Box 780, Haleyville, AL 35565 ATTN: Toys for Kids. Please make all checks payable to Haleyville Rotary Club Toys for Kids.
